This list itemizes the enrollment, exit, reporting and evaluation forms all Fellows must complete and submit to the TEAMS Office. It is important that we receive documents on or before the dates specified so that we can process your Education Award promptly. Some of the forms are in Adobe PDF format, as noted. TEAMS Fellows are required to attend the annual Fall Institute and Pedagogical Seminars held each month in the school year.

Fall Institute is a mandatory pre-service orientation for all Fellows.

Pedagogical Seminars are monthly training workshops designed to help Fellows build teaching skills, address critical issues in multicultural education, and network with peers and experts in the field.
