This list itemizes the enrollment, exit, reporting and evaluation forms all Fellows must complete and submit to the TEAMS Office. It is important that we receive documents on or before the dates specified so that we can process your Education Award promptly. Some of the forms are in Microsoft Excel format, as noted. Download by clicking on the name of the form, open the file in Excel, input information as directed, then email the form back to us at teams@socrates.usfca.edu.
